CONFIDENTIALITY: It is my policy that NO copies of medical records are to be released from the office without the written consent of the patient. Should insurance companies request information with the patient's written consent, I usually try to double check with the patient that they do indeed want a summary sent. I don't send copies of the chart, but instead, write a summary of visits and findings.
When patients request in writing that
records be sent to other doctors, I send a summary and relevant laboratory results. If a particular result is wanted, it can be specifically requested.
I don't discuss patients' conditions with their family members, unless the patient has given permission.
COMMUNICATION: Whenever a test is ordered for a patient, be it a blood test, x-ray, or a special exam, you should receive a report or speak with me directly about the result. Never assume that a test is normal. No news is not good news, but reflects a break down in communication. Usually, I send patients the results of their blood tests with a written comment by me. These should be saved by you for your records. If you don't hear from me after a reasonable period of time, please call.
Because of confidentiality/security reasons, I do not leave test or lab results on message machines or fax machines.
REFERRALS: When the occasion arises where you or I feel that the scope of your problem requires the expertise of a specialist, a referral will be made.
If I ask you to see a specialist for a particular problem, please be sure that the doctor receives my records to expedite their evaluation. Let me know when you will be seeing the specialist. Be sure that the specialist reports to me their findings and recommendations, and clarifies who will continue to monitor the condition. This is your responsibility!
If you are in managed care and are requesting a referral (for a problem not in my field, i.e. ophthalmology); please fax or mail to me your request with the following information:

Name, address, phone and fax number of specialist

the specialist's insurance ID number

name of insurance carrier
-
nature of problem or reason for referral.
(Our office can receive up to 40 requests for referrals per week; the above protocol makes this tedious task easier, thanks).
