GENERAL OFFICE INFORMATION
LOCATION: The office is at 55 Central Park West on the corner of 66th St. near the restaurant Tavern
on the Green.
TRANSPORTATION: The M10 (CPW) and the M66 (cross-town) busses stop on this corner. The 1/9 Subway stop in one block away at 66th Street and Broadway. The Columbus Circle Station, (A,D,B,C trains) is 6 blocks away. There is indoor parking in the building just off CPW with entrances both on 65th and 66th Street. The police
tow illegally or double parked cars in this area.
HOURS: 8:15 AM to 2:00 PM, Monday through Friday. Closed holidays. On occasion I see patients on Saturdays. If you can only be seen on
the weekends, call the office to find which Saturdays are available. Because my schedule can change, you must confirm these appointments the day before. To reach me for problems which cannot wait until the next working day,
call my office number. (see Emergencies below)
INITIAL VISITS: Upon arrival you will be asked to fill out an information form. This provides us with important information regarding your insurance and how we can reach you. Please bring with you:

your health insurance or Medicare card
-
a full list of the medications you are currently taking, including the dose and frequency (bring them with you if it is easier)

the full name, address and phone number of the doctor who may have referred you
-
any relevant medical reports, laboratory or x-ray results, including your immunization history
-
any medical forms that need completion
If you haven't had recent blood testing, I may want to draw a blood sample. For this reason please arrive fasting, i.e. take nothing but water for the preceding 12 hours. If your are taking medications, continue to take them in your usual way.
EMERGENCIES: If you are experiencing an emergency - a severe injury, chest pains, loss of consciousness, or what you feel is a life-threatening illness - call 911 immediately. If it is practical or possible, you should direct the EMS to take you to Roosevelt Hospital on West 59th Street between 9th and 10th Avenue. Call my office as soon as possible.
For situations which are less serious but which require immediate attention, during non-office hours I can be reached through my answering service by calling my regular office number
212-799-0630. If there is no response in what you feel is a reasonable time - call the service again. I carry a beeper and maintain a 24 hour service to notify me for emergencies.I
If I am away, you will be given the name and number of the doctor covering me.
